The Virginia High School League (VHSL) requires each student athlete to have a pre-participation sports physical completed in order to participate in middle or high school sports. The physical must be completed for each school year between May 1st of the current year through June 30th of the succeeding year.
The AAP recommends that your regular provider be the best source of an athletic pre-participation exam. If you are due for a regular checkup we will include the form completion with the checkup. If you have had a regular checkup within the past year, we can fill out a sports physical form for you for a $5.00 fee.
If you need to just schedule a sport physical outside of a regular check up you may do so. The fee currently is $50
A sports physical is required for all public middle and high schools. A Virginia sports physical form can be obtained from your school or by clicking here.
It is most helpful if you complete the medical history portion of the sport physical form prior to your appointment.
